Self-Direct, Inc., NYS DOH Licensed Home Care agency is currently seeking a per diem Registered Nurse, responsible for on-site visits and patient assessments, and/or RN on-call telephone triage during evening, weekend, and holiday hours.

Qualifications:

· Current NYS RN license

· Valid driver’s license

· Two (2) years of home care experience, preferred

· Strong interpersonal, verbal, and written communication skills required

· Computer literacy, including but not limited to data entry, retrieval, and report generation
