Self-Direct, Inc. is a NYS Department of Health (DOH) Licensed Home Care Agency and a certified Women in Business Entity, with offices located in Baldwinsville and Utica, NY. We have been providing licensed home care services since 2003, serving nine counties. We also provide Consumer Directed Personal Assistance Services (CDPAS), Personal Emergency Response Systems (PERS), Home Medical Equipment, and a Social Adult Day Program. We provide services under the NYS DOH Traumatic Brain Injury Waiver, Nursing Home Transition and Diversion Waiver, and contract with NYS managed long-term care (MLTC) programs, private insurance companies, and private pay clients. We currently provide licensed home care services to nine counties in Central New York.

We maintain an individualized plan of care that centers on the rights of the person we are serving. We also collaborate directly with other healthcare professionals to ensure each client is receiving the best possible care at all times, and not at risk of nursing home placement or hospital admission. We are passionate about maintaining and/or improving the quality of life of our clients by consistently assessing the quality of care being delivered.

All caregivers are highly trained, screened and credentialed healthcare professionals. Self-Direct is also approved and authorized by the NYS Department of Health to provide a 40 hour training program for Personal Care Aide (PCA) certification in Syracuse, NY and Utica, NY.

Self-Direct is not only a home care provider. We also own and operate a Social Adult Day Program, which allows individuals to socialize with other adults and participate in a variety of exercise and educational programs. We also provide medical alert systems with fall detection and GPS capabilities, and monitored medication dispensers. We are always looking to expand and improve our services to provide the customize care you deserve and desire.
